Educational Purpose Disclaimer
1. NurseQuake is designed and intended exclusively as a supplementary educational study tool for individuals preparing for the NCLEX-RN and NCLEX-PN licensing examinations and for general nursing knowledge development.
2. The App is NOT a licensed educational institution, accredited nursing program, continuing education provider, or professional development course. Completion of activities within the App does not result in:
– Academic credit of any kind
– Continuing education units (CEUs) or contact hours
– Certification, licensure, or qualification of any kind
– Any credential, certificate, or official documentation
– Official recognition by any educational body, licensing board, or
professional organization
3. NurseQuake is intended to supplement — not replace — formal nursing education, faculty instruction, supervised clinical experience, and accredited preparation programs.
4. The App is designed for individual personal study use only. It is not intended for classroom instruction, institutional licensing, group study sessions, or commercial educational delivery.
5. Students should always prioritize their nursing program’s official curriculum, required textbooks, faculty guidance, and clinical experiences over content encountered in this App.
Not Medical Advice Disclaimer
1. NO MEDICAL ADVICE. Nothing in NurseQuake — including but not limited to quiz questions, answer choices, explanations, rationales, references, flashcard content, dosage calculation examples, ECG rhythm descriptions, lab value ranges, clinical skills steps, or mnemonic content — constitutes, or should be interpreted as, medical advice, clinical guidance, nursing recommendations, diagnoses, treatment plans, prescriptions, or any other form of professional healthcare advice.
2. NO PROFESSIONAL RELATIONSHIP. Use of NurseQuake does not establish any professional relationship between you and NurseQuake, including a nurse-patient, physician-patient, educator-student, or any other professional advisory relationship.
3. NO SUBSTITUTE FOR PROFESSIONALS. Never use NurseQuake content as a substitute for, or in lieu of, professional advice from:
– Licensed physicians, nurse practitioners, or physician assistants
– Registered nurses or licensed practical nurses in supervisory roles
– Pharmacists regarding medication questions
– Your nursing program faculty and clinical instructors
– Hospital or clinical facility protocols and formularies
– Institutional clinical decision support systems and tools
4. SEEK QUALIFIED HELP. Always seek qualified professional advice for any health, clinical, or patient care concern. Do not delay seeking professional advice because of something you read or encountered in NurseQuake.
5. EMERGENCY SITUATIONS. In any medical emergency involving actual patients or persons, call emergency services immediately (911 in the US). Do not consult NurseQuake in emergencies.
Clinical Setting Use Prohibition
THE FOLLOWING IS AN ABSOLUTE PROHIBITION. USE OF NURSEQUAKE IN THESE CONTEXTS COULD ENDANGER PATIENT SAFETY AND EXPOSE YOU TO SERIOUS PROFESSIONAL AND LEGAL CONSEQUENCES.
1. PROHIBITED LOCATIONS. You must NOT use NurseQuake in:
– Any hospital unit, ward, floor, department, or patient care area
– Operating rooms, procedure rooms, or recovery areas
– Intensive care units (ICU, NICU, PICU, CCU, CVICU, or similar)
– Emergency departments or trauma bays
– Long-term care facilities, nursing homes, or hospice settings
– Mental health units, psychiatric wards, or behavioral health facilities
– Outpatient clinics or any setting where patient consultations occur
– Any location where a patient is actively under your care or supervision
2. PROHIBITED ACTIVITIES. You must NOT use NurseQuake to:
– Support, inform, or validate any clinical decision during active care
– Calculate actual medication doses for any real patient
– Interpret ECG/EKG tracings for actual patients in any setting
– Determine appropriate lab value interpretation for a real patient
– Verify procedure steps while performing a clinical skill on a patient
– Provide or support nursing rationale in patient care documentation
– Communicate clinical information to colleagues during patient care
3. WHY THIS MATTERS. NurseQuake content is simplified for NCLEX examination preparation. Clinical care requires nuanced, real-time, patient-specific judgment supported by current evidence, institutional protocols, and professional supervision — none of which NurseQuake provides.
4. PROFESSIONAL ACCOUNTABILITY. As a nursing student or healthcare professional, you are responsible for knowing and adhering to the scope of practice for your licensure status, your institution’s policies, and applicable state/provincial nursing practice acts. NurseQuake is not responsible for consequences arising from your failure to exercise appropriate professional judgment.
Nclex® Trademark And Institutional Affiliation Disclaimer
1. TRADEMARK NOTICE. “NCLEX,” “NCLEX-RN,” “NCLEX-PN,” and “Next Generation NCLEX” (NGN) are registered trademarks and service marks of the National Council of State Boards of Nursing, Inc. (“NCSBN”). These trademarks are used within NurseQuake for descriptive and nominative fair use purposes only — to identify the type of licensure examination for which NurseQuake content may be relevant. Such use does not imply any affiliation with, endorsement by, license from, or sponsorship by NCSBN.
2. NO AFFILIATION. NurseQuake is an independent product developed by an independent developer. NurseQuake has NO affiliation, association, connection, authorization, sponsorship, endorsement, or licensing relationship with any of the following:
– The National Council of State Boards of Nursing (NCSBN)
– Pearson VUE (the NCLEX test delivery vendor)
– Any state board of nursing, territorial board, or provincial regulatory
college of nurses
– Any nursing school, college, university, or educational institution
– Any accreditation body (ACEN, CCNE, CNEA, or others)
– Any nursing textbook publisher (Saunders, Hesi, Kaplan, ATI, UWorld,
Lippincott, Elsevier, or similar)
– Any professional nursing organization (ANA, NLN, AACN, NSNA, or others)
– Any government health agency (FDA, CDC, NIH, WHO, or others)
– Any hospital system or healthcare organization
3. OFFICIAL INFORMATION. For official NCLEX examination information, registration, scheduling, candidate bulletins, test plans, and results, visit NCSBN’s official website at www.ncsbn.org and Pearson VUE’s website at www.pearsonvue.com/nclex.
4. TEST PLAN ALIGNMENT. NurseQuake’s content is designed to align broadly with published NCLEX test plans for study purposes. However, this alignment is independent and not validated, reviewed, or approved by NCSBN. The NCLEX test plan is updated periodically; NurseQuake may not reflect the most current version.
Nclex Readiness Score And Analytics Disclaimer
1. WHAT THE SCORE IS. The “NCLEX Readiness Score” displayed in NurseQuake is a proprietary, internally calculated metric based solely on your performance within the NurseQuake application. It is computed using a formula that weights your in-app accuracy (40%), category coverage breadth (30%), and total question volume (30%).
2. WHAT THE SCORE IS NOT. The NCLEX Readiness Score:
– IS NOT a validated psychometric assessment or standardized test
– IS NOT correlated with, equivalent to, or predictive of actual NCLEX
examination performance, pass probability, or results
– IS NOT endorsed by, validated by, or derived from NCSBN methodology
– IS NOT equivalent to or comparable with any official readiness
assessment (e.g., ATI, HESI, Kaplan, UWorld QBank or self-assessment)
– IS NOT a clinical competency assessment
– DOES NOT account for test anxiety, examination conditions, sleep,
stress, or any real-world factors affecting examination performance
3. HOW TO USE THE SCORE. The Readiness Score should be used exclusively as a self-directed motivational tool to identify areas for further study and to track your progress within the App. It should NOT be used to make decisions about your readiness to sit for the NCLEX examination, to withdraw from your nursing program, to seek additional preparation, or for any other consequential purpose.
4. XP, LEVELS, AND BADGES. Experience points (XP), achievement levels (1-9), streaks, and badges within the App are gamification features for motivational purposes only. They have no educational significance, professional meaning, or correlation with any real-world measure of nursing knowledge or competency.
5. CATEGORY ANALYTICS. Category mastery percentages and strength/weakness identifications reflect only your performance on NurseQuake questions in those categories. They are not validated assessments of clinical competency in those areas.
Ecg/Ekg Content Disclaimer
1. NATURE OF CONTENT. The ECG/EKG rhythm strip identification feature in NurseQuake presents synthetic, computer-generated visual representations of cardiac rhythm patterns. These representations are rendered using SwiftUI drawing paths and algorithms and constitute stylized educational illustrations only.
2. NOT CLINICAL-GRADE. NurseQuake ECG strips:
– Are NOT derived from, based on, or representative of real patient ECG
recordings or clinical data of any kind
– Are NOT calibrated to standard ECG paper speed (25 mm/s), amplitude
(10 mm/mV), or paper format
– Do NOT accurately represent the full morphological variability,
subtlety, or clinical complexity encountered in actual 12-lead or rhythm strip ECGs
– Do NOT represent the artifact, interference, or variation present in
real-world ECG recordings
– Are simplified to present prototypical features for educational
recognition purposes only
– Are NOT suitable for clinical training in ECG interpretation beyond
very basic recognition concepts
3. CLINICAL USE PROHIBITED. NurseQuake ECG strips must NEVER be used:
– For interpreting, diagnosing, or treating actual patients’ cardiac
rhythms or conditions
– As a clinical reference for rhythm identification in any patient care
setting
– To teach bedside ECG interpretation to clinical staff
– For any purpose other than NCLEX examination study
4. SEEK QUALIFIED INSTRUCTION. Proper clinical ECG interpretation requires formal education, supervised clinical experience, and ongoing training. Nurses and nursing students should seek ECG interpretation education through accredited programs, institutional competency training, and supervision by qualified clinical educators.
Dosage Calculation Disclaimer
1. EDUCATIONAL PURPOSE ONLY. All dosage calculation scenarios presented in NurseQuake use hypothetical patients, hypothetical medications, and hypothetical clinical parameters for educational practice of mathematical calculation skills only.
2. ABSOLUTE PROHIBITIONS. NurseQuake dosage calculations:
– MUST NEVER be used to calculate real medication doses for any actual
patient, under any circumstances
– MUST NEVER be used as a reference during actual medication preparation,
dispensing, or administration
– MUST NEVER be used to verify or double-check actual clinical dosage
calculations
– Are NOT validated against current drug package inserts, manufacturer
guidelines, institutional formularies, or pharmacopoeia standards
3. MEDICATION ERRORS ARE DANGEROUS. Medication errors can cause serious harm or death. Actual medication dosing in clinical practice must always:
– Follow your institution’s official medication administration policies
– Be verified against authoritative references (pharmacy formularies,
package inserts, Lexicomp, Micromedex, or equivalent)
– Be checked by a second qualified clinician per institutional protocols
– Be verified with the dispensing pharmacist for any dose that is unclear
or unusual
4. PHARMACOLOGICAL CONTENT. Drug names, classifications, mechanisms, side effects, contraindications, and dosing ranges mentioned in NurseQuake are simplified for educational purposes and may be outdated, incomplete, or inaccurate. Always verify pharmacological information with current, authoritative clinical references before applying any knowledge in clinical practice.
Hospital Code Sound Disclaimer
1. EDUCATIONAL AND MOTIVATIONAL USE ONLY. The hospital code audio tones available in NurseQuake (including Code Blue, Code Red, Code Pink, Code Orange, Code Grey, Code Silver, Code Black, and any other named codes) are synthesized audio files created for use as alarm notification tones for study motivation purposes within the NurseQuake application.
2. NOT EMERGENCY ALERTS. These sounds:
– Are NOT emergency alert systems, mass notification systems, or public
address announcements
– Are NOT affiliated with, produced by, or representative of any real
hospital’s actual emergency code system
– Are NOT certified, regulated, or approved by any healthcare standards
body (JCI, TJC, OSHA, NFPA, or others)
– May NOT match the code sounds, protocols, or meanings used by any
specific healthcare facility (hospital code protocols vary by institution and region)
3. PROHIBITED USE CONTEXTS. You must NOT:
– Use NurseQuake alarm sounds in any clinical or healthcare setting where
the sounds could be mistaken for actual emergency codes
– Play these sounds for others in a deceptive manner intended to create
alarm, confusion, or a simulated emergency response
– Use these sounds in any context where they could interfere with actual
emergency response systems or patient safety
– Use these sounds in any clinical simulation exercise as a substitute
for properly designed simulation equipment and protocols
4. VARY BY FACILITY. Hospital emergency code protocols (what each color code means, how codes are announced, and how staff respond) vary significantly between healthcare facilities, countries, and health systems. NurseQuake’s use of code terminology is illustrative and does not reflect any single facility’s specific protocol.
Alarm And Notification Reliability Disclaimer
1. BEST-EFFORT DELIVERY. NurseQuake alarms and notifications are delivered on a best-effort basis using iOS system services (UserNotifications framework and, where available on iOS 26+, AlarmKit). Delivery depends on numerous factors outside our control, including:
– Your device’s iOS version and model
– Battery charge level and Low Power Mode activation

2. NO GUARANTEE. We CANNOT and DO NOT guarantee that:
– Alarms will sound at the scheduled time
– Notifications will be delivered at all
– Notification content will display correctly
– Quiz-to-dismiss functionality will perform without error
– Snooze functionality will operate as expected
– Follow-up reminder notifications will be sent or received
– AlarmKit alarms will override silent mode in all circumstances
3. DO NOT RELY SOLELY ON THIS APP. You should NOT use NurseQuake as your sole or primary alarm or reminder for:
– Nursing clinical shift start times
– NCLEX examination check-in times
– Class attendance or academic deadlines
– Medication administration for yourself (seek medical guidance for
personal medication schedules)
– Any commitment, appointment, or activity where reliability is critical
4. USE ADDITIONAL ALARM SOURCES. We strongly recommend using multiple alarm methods (including your device’s native Clock app, a physical alarm clock, or asking a household member) for any time-critical commitment.
5. NO LIABILITY. We shall not be liable for any consequence arising from a missed alarm, failed notification, or late or non-delivered reminder of any kind.
Content Accuracy And Currency Disclaimer
1. REASONABLE EFFORTS. NurseQuake makes commercially reasonable efforts to ensure that educational content is accurate, current, and aligned with generally accepted nursing practice standards and NCLEX test plan frameworks at the time of content creation.
2. NO WARRANTY OF ACCURACY. Notwithstanding the above, we CANNOT guarantee and DO NOT warrant that any content in NurseQuake is:
– Accurate, complete, current, or free from errors
– Consistent with the most recent NCLEX test plan (NCSBN updates test
plans periodically)
– Consistent with the most recent clinical guidelines, standards of
practice, or evidence-based protocols
– Consistent with current drug approvals, formularies, or labeling
– Applicable to clinical practice in your specific state, country, or
institutional setting
3. KNOWN LIMITATIONS. You expressly acknowledge the following inherent limitations of NurseQuake educational content:
– Medical and nursing knowledge evolves continuously; content created
at one point may become outdated as standards change
– Content may oversimplify complex clinical topics for NCLEX preparation
– Answer choices and explanations are optimized for NCLEX examination
style, which may differ from nuanced real-world clinical reasoning
– Drug information (names, doses, interactions, contraindications) may
change due to FDA actions, manufacturer updates, or formulary changes
– Laboratory reference ranges vary between laboratories, populations,
patient age groups, and testing methods
– Clinical procedures and nursing interventions differ between
institutions, states, and countries
– NCLEX questions in the App are practice items created by NurseQuake,
not official NCSBN items
4. YOUR RESPONSIBILITY. You are solely responsible for verifying all clinical information encountered in NurseQuake against:
– Your nursing program’s official curriculum materials and faculty
– Current edition accredited nursing textbooks (Saunders, Hesi, etc.)
– Current peer-reviewed clinical guidelines and evidence-based resources
– Current NCSBN NCLEX test plans (available at www.ncsbn.org)

5. ERROR REPORTING. If you believe you have identified an error in NurseQuake content, please report it to support@nursequake.com. We review all error reports but cannot guarantee that all reported issues will be corrected or corrected within any specific timeframe.
App Reliability And Technical Disclaimer
1. PROVIDED AS-IS. NurseQuake is provided on an “as is” and “as available” basis. We do not warrant that the App will be free from bugs, errors, crashes, data corruption, or other technical issues.
2. NO GUARANTEE OF AVAILABILITY. We may need to update, modify, suspend, or discontinue the App (or any feature thereof) at any time without notice. We do not guarantee that the App will be available at any particular time or for any particular duration.
3. DEVICE COMPATIBILITY. NurseQuake requires iOS 16 or later on an Apple iPhone. Not all features may be available on all iOS versions or device models. Certain features (such as AlarmKit) require iOS 26 or later.
4. DATA LOSS RISK. While we design for data reliability, we cannot guarantee against data loss due to device failure, iOS updates, App bugs, or user error. We strongly recommend enabling iCloud Backup on your device.
5. SUBSCRIPTION VERIFICATION. Premium feature access depends on Apple’s StoreKit framework for subscription verification. Technical issues with Apple’s systems may temporarily affect premium feature availability. We are not responsible for Apple platform outages or delays.
No Guarantee Of Examination Passage
1. NO EXAM GUARANTEE. Use of NurseQuake — regardless of subscription tier, questions answered, score achieved, level attained, or time invested — does NOT guarantee or imply any of the following:
– Passing the NCLEX-RN or NCLEX-PN examination
– Passing any other nursing licensure or certification examination
– Receiving a passing score on any nursing school examination or clinical
competency assessment
– Graduating from any nursing program
– Obtaining nursing licensure in any jurisdiction
– Competence in any nursing skill or clinical domain
2. MANY FACTORS AFFECT EXAM RESULTS. NCLEX examination outcomes are determined by many factors that NurseQuake cannot influence, including but not limited to: the quality and depth of your formal education, your clinical experience, your cognitive and test-taking abilities, your physical and mental state on the examination day, the specific adaptive questions selected by the CAT algorithm, and numerous other individual factors.
3. USE OFFICIAL RESOURCES. For official information about NCLEX preparation, always refer to NCSBN’s official Next Generation NCLEX resources, your state board of nursing requirements, and your nursing program’s official preparation guidance.
Independent Professional Obligation
1. AS A NURSING STUDENT or nursing professional, you have independent professional and ethical obligations that exist entirely separate from and superior to any content you encounter in NurseQuake. These include but are not limited to:
– Complying with your state or provincial nurse practice act
– Following your institution’s policies, protocols, and formularies
– Practicing within your licensed scope of practice
– Maintaining professional nursing standards as established by relevant
bodies (ANA, NLN, AACN, or equivalent in your country)
– Seeking supervision and guidance when encountering clinical situations
beyond your current competency

2. NURSEQUAKE DOES NOT MODIFY OR SUPPLEMENT these obligations in any way. Your professional and legal obligations take precedence over anything you have encountered or learned from NurseQuake in all circumstances.
3. SCOPE OF PRACTICE. As a nursing student, you must always practice within your student scope of practice under appropriate supervision. NurseQuake does not grant any additional scope of practice, clinical authority, or authorization of any kind.
Patient Safety And Emergency Services Warning
1. PATIENT SAFETY FIRST. If you encounter a patient safety concern, a deteriorating patient, or any clinical emergency, you must:
– Immediately follow your facility’s emergency response protocols
– Notify appropriate clinical staff and supervisors
– Call for emergency assistance as required (code team, rapid response,
911, or equivalent)
– NEVER stop to consult NurseQuake during any actual emergency
2. EMERGENCY SERVICES. NurseQuake is NOT an emergency service. The App cannot call for help, alert others to an emergency, or provide emergency medical guidance. In any life-threatening emergency, call your local emergency number (911 in the US, 999 in the UK, 112 in the EU, or the applicable emergency number in your country) immediately.
3. ALARM SOUNDS. NurseQuake’s hospital code alarm sounds are for personal study motivation use on your personal device only. They are NOT emergency signals and must NOT be treated as such by you or anyone near you.
